Jeffrey Todd Slavik, DDS

Jeffrey Todd Slavik, DDS, 73, of Sturgeon Bay, passed away at home on November 22, 2025.

Born September 19, 1952, he was a proud lifelong resident of Sturgeon Bay. He graduated from Sturgeon Bay High School in 1970 and attended the University of Wisconsin-Madison before completing dental school at Marquette University in 1977. He was a well-respected dentist for nearly 40 years and was beloved by his patients and staff.

Jeff devoted his life to raising his two kids, was proud to talk about them, and enjoyed every moment he could share with them. In more recent years, he enjoyed reading to his grandkids and taking them to the playground. He loved the simpler things in life, spending time outdoors, planting trees, going out to eat, watching Packers and Brewers games, collecting coins, and eating ice cream. He was an avid animal lover, forming special bonds with his many dogs over the years, and even the smallest of creatures were like pets to him. He will be remembered for his playful sense of humor and his generosity to others.

Jeff was preceded in death by his loving parents, Jerry and Pat Slavik.

His memory will live on in his children, Christopher Slavik and Stephanie Slavik; grandchildren Ellison and Keaton Lauger; siblings, Kim Jacobs and Todd (Angel) Slavik; and nephews and nieces.

In honor of Jeff’s wishes, no formal services will be held. He will be laid to rest in Bayside Cemetery in the Town of Sevastopol.

As a tribute, Jeff would appreciate planting a tree or enjoying a walk and looking at the stars.
